Abstract
Cilj istraživanja je na osnovu prikupljenih podataka koja su vremenski grupirana u četiri desetogodišnja razdoblja uočiti da li na broj učinjenih bronhoskopskih pregleda imaju utjecaj postavljene zdravstvene indikacije, životna dob i spol ispitanika. Želimo i ustanoviti kroz isto promatrano razdoblje koliki je broj učinjenih bronhoskopija i koji je udio između krute i fleksibilne bronhoskopije te broj zabilježenih komplikacija u ovisnosti o korištenoj bronhoskopskoj tehnici. Ispitanici i metode. Ispitanici su hospitalizirana djeca koja su se od razdoblja 1978. do 2018 godine liječena na Klinici za pedijatriju KBC Rijeka, a da im je tijekom hospitalizacije učinjen bronhoskopski pregled. Ukupno je bilo 1138 ispitanika koji su grupirani u 4 grupe po desetogodišnjim razdobljima (Grupa 1 1978-1987; Grupa 2 1988-1997; Grupa 3 1998-2007; Grupa 4 2008-2018. Dob ispitanika se kretala u rasponu od 2 dana do 18 godina (medijan = 3 godine). U ispitivanju je bilo 652 dječaka i 486 djevojčica. Podaci su prikupljeni iz arhivirane medicinske dokumentacije, integriranog bolničkog informacijskog sustava (IBIS), operacijskih protokola i knjige protokola za bronhoskopije. Promatrani su slijedeći parametri: dob (dojenačka, predškolska i školska dob), spol i indikacije za izvođenje bronhoskopskog pregleda. Posebnu promatranu skupinu čine ispitanici koji su podvrgnuti ponovljenim bronhoskopskim pregledima. Rezultati istraživanja: U sve četiri ispitivane grupe najveći broj ispitanika su bila muška djeca predškolskog uzrasta. Iz objektivnih razloga indikacije za fleksibilnu bronhoskopiju nismo mogli usporediti između Grupa 1 i 2 i Grupa 3 i 4 iz razloga što se do 1999 godine u Klinici za pedijatriju KBC Rijeka nije primjenjivala fleksibilna bronhoskopija. Ispitanici u Grupama 1 i 2 bili su podvrgnuti samo krutom tipu bronhoskopskog pregleda, dok su u ispitanika Grupa 3 i 4 bile primjenjivane obje bronhoskopske tehnike. Ipak u zadnjem promatranom desetljeću (Grupa 4) omjer korištenja fleksibilne i krute bronhoskopije se skoro izjednačio s još uvijek malom prevagom uporabe krute bronhoskopije što je svakako uvjetovano postavljenim indikacijama. Indikacije za bronhoskopski pregled variraju u odnosu na desetljeća u kojima su postavljene, ali u sve četiri grupe među najčešćim indikacijama za bronhoskopski pregled bila je ona zbog sumnje na aspiraciju stranog tijela. Zaključak: Kod bolesti pluća i dišnih putova u djece bronhoskopija kao endoskopska metoda koristi se u dijagnostičke i terapijske svrhe. Indikacije utječu na izbor bronhoskopske tehnike.
Abstract (english)
The aim of the study, based on data collected over the span of four decades, was to find out whether the number of bronchoscopic examinations carried out is affected by the participants' health indications, age, and gender. For the same period we want to determine the number of bronchoscopies performed, the proportion between rigid and flexible bronchoscopies, and the number of complications reported depending on the bronchoscopic technique. Subjects and methods: The sample comprised hospitalized children who were treated at the Pediatric Clinic of CHC (Clinical Hospital Centre) Rijeka from 1978 to 2018 and have undergone a bronchoscopic examination. There were 1138 participants in total which were divided into four groups according to ten-year periods (Group 1 1978–1987; Group 2 1988–1997; Group 3 1998–2007; Group 4 2008–2018). The participants' age range is between two days and 18 years (median = 3 years). A total of 652 boys and 486 girls participated in the study. Data were collected from archived medical records, the integrated hospital information system (HIS), operation protocols, and bronchoscopy protocol books. The following parameters were observed: age (infant, preschool, school), sex, and indications for the performance of a bronchoscopic examination. Participants who have undergone repeated bronchoscopic examinations were observed separately. Results: The largest number of participants in all four groups observed were male preschool children. Since before 1999 flexible bronchoscopy was not performed at the Pediatric Clinic of CHC Rijeka, for objective reasons indications for flexible bronchoscopy between groups 1 and 2 and 3 and 4 could not be compared. Participants in groups 1 and 2 have undergone only the solid type of bronchoscopic examination, while participants in groups 3 and 4 have undergone both types of bronchoscopic procedure. However, in the last decade observed (Group 4) the ratio between the use of flexible and rigid bronchoscopy and the still slightly predominant rigid bronchoscopy was almost even, likely as a result of set indications. Indications for a bronchoscopic examination vary with respect to the decade in which they were set, but in all four groups one of the most common indications for a bronchoscopic procedure was the suspicion of a foreign body aspiration. Conclusion: In lung and respiratory diseases in children bronchoscopy as an endoscopic method is used for diagnostic and therapeutic purposes. Indications influence the choice of bronchoscopic technique.
