Abstract
Rak je jedan od najvećih zdravstvenih problema sadašnjice. Rak (karcinom) želudca je bolest sa visokom smrtnošću. U završnome radu prikazani su karcinomi želudca i njihova klasifikacija te najčešćedijagnostičkepretrage. Također su prikazane metode liječenja koje se dijele na kirurško i sistemsko liječenje karcinoma želudca. U dijelu specifične zdravstvene njege prikazana je uloga medicinske sestre kod pacijenta nakon totalne gastrektomije. Veliku važnost nakon operacije uklanjanja želudca potrebno je posvetiti pravilnoj prehrani. Uspostavljanje režima pravilne i uspješne prehrane kod pacijenta nakon totalne gastrektomije od velikog je značaja jer je velika vjerojatnost da se pacijenti, poglavito nakon otpusta iz bolnice u procesu prehrane neće snaći, oklijevati će tražiti pomoć čime postoji mogućnost nastanka pothranjenosti i mortaliteta. Kako je više od 75% bolničkih pacijenata, posebice onih koji u njoj provode višeod pet dana, ovisno o bolničkoj prehrani, mogu postati pothranjeni, izgubiti na težini, imatinarušene funkcionalnesposobnosti, produljiti oporavak nakon liječenja i povećati brojkomplikacija te povećati troškove liječenja.Pacijentima je posebno važnospriječiti razdoblje gladovanja, jer je normalna adaptivna reakcija organizma na gladovanjesmanjena zbog razvoja metaboličkih promjena uslijed kroničnog stresa bolesti. Nutritivno liječenje pacijenta nakon gastrektomije vrlo je važno i planira se iprilagođava individualno prema težini metaboličkih promjena. Prilikom planiranja nutritivne potpore uzimaju se u obzir specifičnosti pacijenta i bolesti.U ovom završnom radu provedeno je istraživanje o učincima ranog oralnog hranjenja nakon gastrektomije na postoperativni oporavak bolesnika.
Abstract (english)
Cancer is one of the biggest health problems of our time. Gastric cancer is a disease with ahigh mortality rate. The final paper presents gastric cancers and their classification, as well as the most common abdominal diagnostic procedures. Methods of treatment are also presented, which are divided into surgical and systemic treatment of gastric cancer. In the part of specific health care, the role of the nurse is shown in the patient care after total gastrectomy. After total gastrectomy great importance must be dedicated to proper nutrition. Establishing a proper and successful diet for the patient after a total gastrectomy is of great importance because it is likely that patients, especially after discharge from the hospital, will not cope and they will hesitate to seek help in the dietary field, which leads to malnutrition and mortality. More than 75% hospital patients, especially those who spend their more than five days, are depending of hospital diet, so they can become malnourished, lose weight, have damaged functional abilities, extend recovery after treatment, increase number of complication and costs of treatment. It is especially important for patients to prevent period of starvation because normal adaptive reaction of organism on starvation is reduced due to development of metabolic changes caused by cronical illness stress. Nutritionaltreatment after gastrectomy is very important, it is planned and adjusted individuallyaccordingto heaviness of metabolic changes. Specificity of patients and illness must be considered when planning nutritional support.In this final paper we have research on effects of early oral feeding after total gastrectomy onpostoperative recover of a patient.
