Abstract
CILJ RADA: je utvrditi razinu antenatalne skrbi i usporediti ih s ishodom trudnoće i stanjem novorođenčeta METODE: Istraživanje je provedeno u lipnju 2018. u KBC-u Split na Klinici za ženske bolesti i porode uvidom u medicinsku dokumentaciju (trudničke knjižice i liste novorođenčeta) te popunjavanjem unaprijed pripremljenog obrasca za uzimanje podataka. Obuhvaćeno je 50 babinjača i njihove novorođenčadi 51 (1 blizanačka trudnoća) za vrijeme hospitalizacije koje su slučajnim odabirom ušle u ispitivanu skupinu. REZULTATI: Izrazito je mali udio ispitanica u dobi od 20-24 godine koja je najoptimalnija za rađanje. U samo 6% ispitanica je evidentirana navika pušenja. Od kroničnih bolesti najzastupljenije su endokrinološke bolesti hipotireoza i dijabetes, dok u trudnoći je najzastupljenija preeklampsija, ureaplazma i gestacijski dijabetes. Prosjek broja učinjenih pregleda tijekom trudnoće je 10,34, te je isti prosjek napravljenih ultrazvučnih pregleda što je znatno veći broj od preporučenog. Dokazano je da se uz svaki pregled mjeri tjelesna težina, tlak i provjera urina na bjelančevine što dokazuje prosjek od 9,86 puta napravljenih mjerenja tlaka i kontrole urina, te prosjek 9,88 mjerenja tjelesne težine. Osnovne laboratorijske pretrage, kao i GUK rade se rutinski svakoj trudnici. OGTT je napravljen u 90% ispitanica. Čak 60% ispitanica nije napravilo biokemijske testove na kromosomopatije iako su neinvazivni. Na terminu (38-42 tjednu trudnoće) je rodilo 94% ispitanica, u 74% ispitanica porod završio spontano. Najviše novorođenčadi je bilo u kategoriji težine 3.000-3.500 grama 37% i sa najvećom ocjenom zrelosti po APGAR-ovoj (8-10) koju je imalo čak 98% novorođenčadi. ZAKLJUČAK: Ovim istraživanjem je potvrđeno da se provodi zadovoljavajuća antenatalna skrb (po preporukama Programa mjera za zdravstvenu zaštitu) što dokazuje broj napravljenih pregleda i ultrazvučnih pregleda. Skup svih intervencija u antenatalnoj skrbi dovode do željenog pozitivnog ishoda, što nam pokazuje stanje i procjena novorođenčeta nakon poroda.
Abstract (english)
AIM: The aim of this paper is to examine the level of antenatal care and pregnancy outcome. METHODS: The data was collected in June 2018, in Department of Gynecology and Obstetrics, University Hospital Split. It was based on the medical records (pregnancy book and newborn list) and collected in a form created for this purpose. The study includes random sample of 50 puerperal women and their 51 newborns (1 twin pregnancy) during their hospitalization. RESULTS: The percentage of women aged 20 to 24, an optimum age for having a baby, is very low. Only 6% of the women reported tobacco use. Among chronical diseases, the most common ones are endocrine disorders, hypothyreosis and diabetes. During pregnancy, the most common disorders are preeclampsia, ureaplasma, and gestational diabetes. Women underwent 10.34 pregnancy examinations in average and the same number of ultrasounds was performed on every women in average, which largely exceeds the number of ultrasounds that are recommended during pregnancy. The research has confirmed that every pregnancy examination involves measuring of weight and blood pressure, and testing urine as every pregnant woman had 9.86 blood pressure measurements and urine tests during their pregnancy, as well as 9.88 weight measurements. Basic laboratory tests and glucosa test are a part of a routine in pregnancy. Oral glucose tolerance test was done in 90% of the pregnant women. High rate 60% of the pregnant women did not have biochemical tests for chromosomopathy. In 94% delivery occurred to a due date (38th to 40th week of pregnancy), 74% had a spontaneous delivery. Most newborns (37%) weighted 3,000-3,500 grams and 98% had the highest Apgar score (8-10). CONCLUSION: The number of antenatal visits and ultrasounds confirms that antenatal care is provided adequate (and in accordance with the guidelines of Health care measures). The set of medical interventions in antenatal care has proven to have positive results, as confirmed by examination of the newborns after their birth.
