Abstract UVOD: Periferna arterijska bolest i dijabetes melitus glavni su uzroci amputacije donjih
ekstremiteta u cijelom svijetu, a činjenica da trenutno više od 463 milijuna ljudi diljem svijeta
živi s dijabetesom te 202 milijuna ljudi s perifernom arterijskom bolesti, dodatno naglašava
globalno opterećenje ovih bolesti.
CILJ: Cilj istraživanja bio je prikazati najčešće indikacije za amputaciju donjih ekstremiteta na
Zavodu za vaskularnu kirurgiju s intenzivnom njegom Kliničkog bolničkog centra Rijeka u
vremenskom periodu od 2019. do 2021. godine, te istražiti učestalost amputacija donjih
ekstremiteta obzirom na razinu amputacije i dob ispitanika.
METODE: U istraživanje su uključena 793 pacijenta kod kojih je u navedenom periodu
provedena amputacija donjih ekstremiteta, a iz istraživanja su isključeni svi pacijenti kod kojih
je provedena amputacija gornjih ekstremiteta na svim razinama i pacijenti kod kojih je
provedena amputacija donjih ekstremiteta u drugim ustanovama. Svi podaci potrebni za
ostvarivanje istraživanja prikupljeni su iz bolničkog informatičkog sustava, a statistička obrada
podataka provedena je pomoću programa Statistica (Version 13.5.0.17, 1984-2018 TIBCO
Software Inc).
REZULTATI: U istraživanje je uključeno 72% muškaraca te 28% žena, a najveći broj ispitanika
(59%) bio je stariji od 71 godine. Najčešća indikacija za amputaciju donjih ekstremiteta bila je
periferna arterijska bolest u kombinaciji s dijabetesom melitusom (58%), zatim periferna
arterijska bolest kao samostalna indikacija (31%). Ispitanicima je amputacija donjih
ekstremiteta najčešće provedena na razini prstiju (46%), zatim na razini iznad koljena (35%) te
ispod koljena (15%). Najčešći komorbiditeti među pacijentima uključenim u istraživanje bili
su kardiovaskularne bolesti (42,29%), dijabetes melitus (35,22%) i bolesti bubrega (11,54%).
Više od polovice ispitanika (55%) nije bilo u mogućnosti samostalno provoditi postupke
samozbrinjavanja u trenutku otpusta iz bolnice, već je uvelike ovisilo o pomoći druge osobe,
no unatoč tome se nakon amputacije najviše ispitanika (79%) vratilo u vlastiti dom.
ZAKLJUČAK: Amputacije donjih ekstremiteta najčešće se provode na razini prstiju stopala,
uslijed periferne arterijske bolesti u kombinaciji s dijabetesom melitusom. Najveća prevalencija
istih uočena je kod muškaraca starijih od 70 godina.
Abstract (english) INTRODUCTION: Peripheral arterial disease and diabetes mellitus are the main causes of
lower limb amputation worldwide, and the fact that currently more than 463 million people
worldwide live with diabetes and 202 million people with peripheral arterial disease further
emphasizes the global burden of these diseases.
OBJECTIVE: The aim of the research was to show the most common indications for
amputation of the lower extremities at the Department of Vascular Surgery with intensive care
of the Rijeka Clinical Hospital Center in the period from 2019 to 2021, and to investigate the
frequency of amputations of the lower extremities regarding the level of amputation and the
age of the subjects.
METHODS: The study included 793 patients who underwent lower limb amputation in the
specified period, and all patients who underwent upper limb amputation and patients who
underwent amputation in other institutions were excluded from the study. All the data needed
for the research were collected from the hospital's IT system, and statistical data processing was
carried out using the Statistica program (Version 13.5.0.17, 1984-2018 TIBCO Software Inc).
RESULTS: 72% of men and 28% of women were included in the research, and the largest
number of respondents (59%) was older than 71 years. The most common indication for lower
limb amputation was peripheral arterial disease in combination with diabetes mellitus (58%),
followed by peripheral arterial disease as an independent indication (31%). Among the
respondents, the amputation of the lower extremities was most often performed at the level of
the fingers (46%), then at the level above the knee (35%) and below the knee (15%). The most
common comorbidities among the patients included in the study were cardiovascular diseases
(42.29%), diabetes mellitus (35.22%) and kidney diseases (11.54%). More than half of the
respondents (55%) were not able to independently carry out self-care procedures at the time of
discharge from the hospital but were largely dependent on the help of another person, but
despite this, most of the respondents (79%) returned to their own home after the amputation.
CONCLUSION: Amputations of the lower extremities are most often performed at the level of
the toes, due to peripheral arterial disease in combination with diabetes mellitus. The highest
prevalence of them was observed in men over 70 years old.
