Abstract
Periferna CT angiografija je pretraga koja danas ima široku primjenu u kliničkoj praksi zahvaljujući razvoju tehnologije u zadnjih dvadesetak godina. Uvođenje multidetektorskih CT uređaja (MDCT) je omogućilo primjenu periferne CT angiografije sve češće, što se može pripisati trima svojstvima MDCT-a: njegovu mogućnost da proizvodi veliku količinu volumetrijskih podataka u jako kratkom vremenu, visokoj rezoluciji, mogućnost stvaranja multiplnaranih i trodimenzionalnih (3D) rekonstrukcija koje su od velikog dijagnostičkog značaja pruţajući uvid u sitne anatomske detalje. CT angiografija je postkontrastna pretraga, a osnovni faktori koji utječu na poboljšanje prolaska kontrasta kroz tijelo pacijenta su stanje pacijenta (teţina, srčana funkcija), trajanje injiciranja, brzina injiciranja, volumen kontrasta, koncentracija kontrasta i ispiranje fiziološkom otopinom. Magnituda vaskularne i parenhimalne opacifikacije kontrasta se povećava sa širim vremenskim prozorom ţeljene razine opacifikacije kontrasta. Postoje i opasnosti prilikom korištenja koje se odnose na bubreţnu funkciju te zahtjevaju posebnu paţnju. Najveća prednost periferne CT angiografije se očituje u mogućnosti skeniranja arterijskog stabla cijelom njegovom duţinom od bubreţnih arterija pa sve do najsitnijih arteriola noţnih prstiju. Važan element ove pretrage je i naknadna obrada podataka, koristeći suverene rekonstrukcijske kernele i digitalnu obradu na radnoj stanici. Interpretacija nalaza periferne CT angiografije podrazumijeva detaljno poznavanje anatomije krvnih žila, fizioloških varijacija te patoloških promjena. Perfierna CT angiografija se najčešće koristi za evaluaciju pacijenata s rizičnim faktorima koji dovode do periferne arterijske bolesti, kako bi se uočile i najsitnije patološke promjene, što smo potvrdili i u istraživanju provedenom kao sastavnom dijelu ovog rada, čiji podaci se temelje se na retrospektivnoj analizi 75 bolesnika na Kliničkom zavodu za radiologiju u Rijeci.
Abstract (english)
Peripheral CT angiography is a medical search which is nowadays widely used in clinical practice because of great development of technology during past 20 years. Introduction of multidetector CT devices (MDCT) contributed to the great use of peripheral CT angiography more frequently due to three important performances of multi detector CT devices: their potential to produce large amount of volumetric data very fast, high resolution and their potential to create multiplanar and threedimensional (3D) reconstructions which have great diagnostic signifance in providing small anatomic details. CT angiography is a post-contrast search, basic factors which affect improvement of contrast flow through the body of a patient are condition of a patient (body weight, heart function), duration of injection, rate of injection, contrast volume, concentration of contrast and saline flush. Vascular magnitude and parenchymal opacification of contrast increase with the use of wider time window with desired level of contrast enhacement. Furthermore, there are some health risks related to the renal function that require special attention. The greatest advantage of peripheral CT angiography is the ability of scanning the whole length of arterial tree, all the way from renal arteries to arterioles of toes. One important element of this search is the post-processing of the image with the use of reconstruction kernels and digital processing on the work station. Analysis of obtained peripheral CT angiography images requires good and detail knowledge of blood vessel anatomy, physiological variations and pathological changes. Peripheral CT angiography is mostly used for the evaluation of patients with the risk factors which are the cause for developing peripheral artery disease (PAD), and so that we can notice even the slightest pathological changes.
