Abstract
Izvanmaternična trudnoća je svaka trudnoća koja se implantira van šupljine maternice i predstavlja po život opasno stanje. U vrlo malom broju završava živim plodom, a drugi je uzrok smrti trudnica. Stoga je pravovremena i točna dijagnoza ključna za ishod trudnoće. Simptomi trudnoće su različiti i otežavaju dijagnostiku upućujući na razna opstetrička i ne opstetrička stanja. Glavni simptomi su bol, izostanak menstruacije i krvarenje koji upućuju na abnormalnosti rane trudnoće. Žene sa prethodnim izvanmaterničnim trudnoćama, preboljenim upalnim bolestima zdjelice i žene koje su zanijele postupcima medicinski potpomognute oplodnje, imaju visok rizik od nastanka izvanmaternične trudnoće. Ključ uspješne dijagnoze i ishoda je uvijek imati na umu mogućnost izvanmaternične trudnoće. Transvaginalni ultrazvuk i biokemijski parametri (humani korionski gonadotrpin i progesteron) ključni su za postavljanje dijagnoze. Liječenje ovisi o nizu čimbenika: reprodukcijskoj dobi žene, gestacijskoj dobi, općem stanju, kliničkom nalazu pri prijemu. Metode liječenja izvanmaternične trudnoće su ekspektativna, medikamentna i kirurška. Najčešće se liječenju izvanmaternične trudnoće pristupa kirurški. Od kirurških metoda najčešće se primjenjuje laparoskopija. Laparoskopske metode su manje invazivne što omogućuje ženama brži i bolji oporavak od laparotomijskih zahvata. Primaljska skrb započinje po primitku žene s dijagnozom ektopične trudnoće te zbog najčešćeg kirurškog liječenja primalja obavlja preoperativnu i poslijeoperacijsku skrb za pacijentice.
Abstract (english)
Ectopic pregnancy is every pregnancy implanted out of uterine cavity and represents life threathening condition. A very small number of pregnancys ends with fetus surviving , and it is the second cause of pregnancy deaths. Therefore, timely and accurate diagnosis is crucial for the outcome of pregnancy. The symptoms are diferent and complicate the diagnostics by moving to obstetric and non obstetric states. The main symptoms are pain, lack of menstruation and bleeding that point to abnormalities of early pregnancy. Women with previous ectopic pregnancy, over inflamatory pelvic diseases and women that got pregnant with procedures of medically assisted maternity, have a high risk of ectopic pregnancy. The key to successful diagnosis and outcome of pregnancy is to always keep on mind the possibility of ectopic pregnancy. Transvaginal ultrasound and biochemical parameters are crucial for diagnosis. Treatment depends on variety of factors: female reproductive age, gestation age, general condition, clinical findings at the first examination. Treatment methods are expectative, medicated and surgical. The most common treatment is surgical. Of the surgical methods, laparoscopy is most often used. Laparoscopic methods are less invasive, allowing women faster and better recovery from laparotomy. Midwifes care begins with a diagnosis of ectopic pregnancy and because of the most frequent surgical treatment of the midwife performs preoperative and postoperative care.
