Abstract
Scintigrafija i SPECT/CT paratireoidnih žlijezda s 99mTc-MIBI je dijagnostička nuklearno-medicinska slikovna metoda koja se izvodi kod pacijenata kod kojih se sumnja na poremećaje paratireoidnih žlijezda poput adenoma, hiperplazije ili karcinoma ili kod pacijenata koji su upućeni na operativni zahvat, zbog točnije lokalizacije. U radu je objašnjen način izvođenja pretrage, priprema pacijenta i radiofarmaka te proces snimanja. Učinjena je retrospektivna analiza podataka i nalaza 50 ispitanika sa sumnjom na hiperparatireoidizam koji su upućeni, uglavnom zbog povišene vrijednosti PTH-a, u Klinički zavod za nuklearnu medicinu Kliničkog bolničkog centra Rijeka na scintigrafiju. Ustanovljena je učestalost slanja pacijenata na pretragu prema spolu i životnoj dobi, vrijednosti PTH, uputnoj dijagnozi te konačnom nalazu pretrage. Zaključeno je da su scintigrafija i SPECT/CT paratireoidnih žlijezda ključni u otkrivanju poremećaja doštitnih žlijezda te služe i kao preoperativne metode. SPECT/CT daje daleko superiornije dijagnostičke informacije, otkriva veći broj uvećanih žlijezda i omogućuje njihovu točnu lokalizaciju.
Abstract (english)
Scintigraphy and SPECT/CT parathyroid glands with 99mTc-MIBI is diagnostic nuclear-medical imaging method that is used in patients suspected for disorder of parathyroid gland such as adenoma, hyperplasia, carcinoma or patients referred to surgery for more precise localization. In this work performance of the procedure, patient and radiofarmaceutical preparation and examination process are explained. Retrospective analysis was performed along with the results of 50 examinees with suspicion on hyperparathyroidism who were referred mostly because of high PTH levels in Clinical Deparment of Nuclear Medicine, Clinical Hospital Centre Rijeka. Their gender and age, levels of PTH, referral diagnosis and their final examination results were showed. It is concluded that scintigraphy and SPECT/CT parathyroid glands are vital in detection of parathyroid gland disorders and are used as preoperative methods. SPECT/CT gives far superior diagnostic information, discovers larger amounts of enlarged glands and allows their precise localization.
