Abstract
Križobolja je najčešći razlog traženja medicinske intervencije, drugi je po redu uzrok izostajanja s posla i povezana je s visokim direktnim i indirektnim troškovima, pa je veliki zdravstveni, socijalni i ekonomski problem koji utječe na kvalitetu života. Konzervativno liječenje boli u lumbalnoj kralježnici tradicionalno je empirijski te je kratkoročni cilj smanjenje boli. Nakon prestanka akutne boli slijedi prava rehabilitacija fizioterapijom.Cilj fizioterapijskog tretmana trebao bi biti potpuni oporavak pacijenta i povratak optimalnoj razini funkcioniranja i sudjelovanja u društvu. Sekundarni cilj fizioterapije je prevencija kroničnih problema i ponavljanje boli u donjem dijelu leđa. Liječenje fizikalnim metodama svrstavamo u obavezne postupke preporučenih načina liječenja lumbalnog bolnog sindroma. Ovim metodama poboljšavamo učinke konvencionalnih lijekova za liječenje križobolje, te znatno odgađamo moguće operacijsko liječenje. Učinak pojedinih metoda fizikalne terapije, u individualiziranom pristupu bolesniku, često omogućuje rješenje problema. Za ublažavanje i liječenje križobolje, liječnici se koriste umjerenim analgeticima koji se mogu kupiti bez recepta, do lijekova posebno namjenjenih određenim bolestima. Ukoliko konzervativno liječenje ne pokaže rezultate, odlučuje se za operaciju uz prethodnu neurokiruršku obradu. Cilj ovog istraživanja je prikazati razliku u 5 operativnih skupina s obzirom na broj pacijenata po dobi, spolu, trajanju operativnog zahvata i broju postoperativnih dana provedenih u bolnici. Koristili smo statističku obradu podataka uz pomoć statističkog programa Statistica 13.3 (StatSoft Inc., Tulsa, SAD). U našem istraživanju od 116 ispitanika pokazalo se kako se lumbalni bolni sindrom javlja znatno ranije nego što je definirano u literaturi, možemo pretpostaviti da je razlog tome smanjena fizička aktivnost, prekomjerno sjedenje te pojava degenerativnih promjena na kralježnici (osteporoza). Pokazalo se da je najčešći razlog za operativni zahvat u većini pacijenata stenoza spinalnog kanala te hernija diska u različitim fazama.
Abstract (english)
Low back pain is the most common cause of medical intervention, the second cause of the absence of work and is associated with high direct and indirect costs, and is a major health, social and economic problem affecting the quality of life. Conservative treatment of pain in the lumbar spine is traditionally empirical and short-term goal of reducing pain. After the end of acute pain, real rehabilitation with physiotherapy is followed. The goal of physiotherapy treatment should be to complete patient recovery and return to an optimal level of functioning and participation in society. The secondary goal of physiotherapy is to prevent chronic problems and repetitive pain in the lower back. Treatment by physical methods is included in mandatory procedures of recommended ways of treating lumbar pain syndrome. By these methods we improve the effects of conventional drugs for the treatment of acute myocardial infarction, and considerably delay the possible surgical treatment. The effect of individual methods of physical therapy in an individualized approach to the patient often makes it possible to solve the problem. To relieve and treat acute pain, doctors are using moderate analgesics that can be purchased without prescription, to medicines specifically intended for certain diseases. The aim of this study is to show the difference in 5 operative groups considering the number of patients by age, gender, duration of operation and the number of postoperative days spent in hospital. We used statistical data processing with the help of Statistical Statistics Statistics 13.3 (StatSoft Inc., Tulsa, USA). In our research with 116 patients it has been shown that lumbar pain syndrome occurs much earlier than defined in the literature, we can assume that the reason for this is reduced physical activity, excessive sitting and the appearance of degenerative changes in the spine (osteorrosis). It has been shown that the most common cause for surgery in most spinal canal stenoses and herniated disks at different stages.
