Abstract
Gastrično hranjenje je najčešći način enteralnog hranjenja. Perkutana endoskopska gastrostoma je jeftinija, manje invazivna metoda, bez potrebe za općom anestezijom u većini slučajeva te se smatra boljim izborom od kirurškog postavljanja gastrostomalne cijevi. Nazoenterične sonde (nazogastrična, nazoduodenalna i nazojejunalna) obično se postavljaju kod kratkotrajnog enteralnog hranjenja u bolesnika s očuvanim zaštitnim refleksima dišnih puteva. U usporedbi s perkutanom endoskopskom gastrostomom nazoenterične sonde rezultiraju s više komplikacija, manjim subjektivnim osjećajem udobnosti te nižom učinkovitošću hranjenja. Iz tih razloga se uvođenje perkutane endoskopske gastrostome razmatra kod pacijenata s rizikom od umjerene do teške malnutricije unutar 2-3 tjedna hranjenja nazoenteričnom sondom. Za djelotvornu enteralnu prehranu potreban je multidisciplinaran pristup. Tim sačinjavaju gastroenterolozi endoskopičari koji su odgovorni za postavljanje gastrostome, medicinske sestre, nutricionisti, članovi obitelji te u slučaju potrebe psiholozi. Medicinske sestre pružaju emocionalnu podršku pacijentu, zadužene su za edukaciju pacijenta i obitelji o toaleti gastrostomalne cijevi i hranjenju uz poticanje samostalnosti i aktivnog sudjelovanja bolesnika. Skrb medicinske sestre o pacijentu na enteralnoj prehrani odnosi se na procjenu općeg stanja, nutritivnog statusa i laboratorijskih rezultata. Po izvršenoj procjeni potrebno je izraditi plan zdravstvene njege i provoditi specifične intervencije prilagođene pacijentu. U edukaciju je potrebno uključiti obitelj bolesnika i/ili bliske osobe koje skrbe o pacijentu u svrhu pripreme za nastavak enteralne prehrane u domu bolesnika.
Abstract (english)
Gastric feeding is the most common method of enteral feeding. Percutaneous endoscopic gastrostomy is a cheaper, less invasive method, without the need for general anesthesia in most cases. It is considered a better choice than surgical placement of a gastrostomal tube. Nasoenteric probes (nasogastric, nasoduodenal and nasojejunal) are usually placed during short-term enteral feeding in patients with preserved airway protective reflexes. Compared to gastrostomy, nasoenteric probes result in more complications, less subjective sense of comfort, and lower feeding efficiency. For these reasons, the placement of gastrostomal tube is considered in patients at risk of moderate to severe malnutrition within 2-3 weeks of nasoenteric tube feeding. A multidisciplinary approach is required for effective enteral nutrition. The team consists of endoscopic gastroenterologists who are responsible for setting up gastrostomal tube, nurses, nutritionists, family members, and if necessary psychologists. Nurses provide emotional support to the patients, they are in charge of educating the patient and the family about the tube care and feeding, while encouraging independence and active participation of patients. The nurses´ care of the patient on enteral nutrition includes the assessment of the general condition, nutritional status and laboratory results. After the assessment, it is necessary to develop a health care plan and implement interventionts specific to the patient. The education should include the patient´s family and/or a close persons caring for the patient in order to prepare for the continuation of enteral nutrition in patient´s home.
