The stress-rest protocol occupies one of the key roles in the early assessment and follow-up of patients with ischemic heart disease, providing insight into the risk stratification of serious cardiovascular events and directing the needs for invasive imaging. This diagnostic procedure enables precise detection of the infarcted area and myocardial perfusion defects in the form of inducible ischemia with very high accuracy, providing valuable information about the function and blood supply of the heart muscle. Purpose: Depending on gender and age, to determine the ratio of patients subjected to the cardiac stress-rest MRI protocol, and to determine the percentage of patients in whom inducible ischemia and fibrosis were detected. Methods: The research included a group of 195 patients who, in the period of 02.05.2022. until 02.05.2023., were subjected to the stress-rest protocol of heart MRI at the Clinical Institute of Radiology KBC Rijeka - locality Sušak and locality Rijeka, using a Siemens Magnet Aera 1.5T device. Results: Out of 195 patients, 116 (59%) were male and 79 (41%) were female. More than half of the patients (77.44%) were older than 60 years. Inducible ischemia was found in 33 (16.92%) patients, while cardiac fibrosis was detected in 72 (37%) patients. Conclusion: In this study, it is determined that men older than 60 years are subjected to stress-rest protocol of MR heart significantly more often than women, while inducible myocardial ischemia and fibrosis were found in less than 50% of patients.
